Montefiore Health System is an academic health system that delivers coordinated, compassionate, science-driven care where, when and how patients need it most. Montefiore consists of eight hospitals and an extended care facility, a School of Nursing, and primary and specialty care provided through a network of more than 150 locations across the region, including the largest school health program in the nation and a home health program.
Project Details
I worked closely with Montefiore to create a personalized easy-to-use patient portal that aligned with the company’s branding guidelines. This portal allowed patients to have real-time access to view and track their medical information, communicate with providers, and collaborate in their health. The new design guided patients down clear paths to manage their medical issues and focused on forming loyal relationships between doctors and patients using a dynamic online experience.
